  • 1. 241 W. 56th Ave · Denver CO 80216 · Phone 303-297-9779 · Fax 303-297-0493 Effectiveness of my job performance I began working for US Transport Inc. in January 2009 as the Director of Human Resources. I assumed the additional responsibilities of the Safety function in 2010. The Owner/CEO determined that a full time safety manager was needed. In 2011 a Human Resources Director was hired and I became the full time Safety Manager, (with a title of the VP of Safety), reporting directly to the Owner/CEO. The following activities reflect the favorable financial impact of the Safety functions’ contribution to the profits of US Transport Inc. Claims The “bottom line” for US Transport Inc.’s safety program is the number and severity of claims, both vehicle and worker’s compensation. The goal for US Transport Inc. is to have zero claims which translates to zero injuries to the employees and zero equipment damage. Reviewing the activity for:  Vehicle claims: Policy Year No. of Claims Costs 7/08-6/09 45 $417,740 7/09-6/10 26 $198,077 7/10-6/11 24 $713,787 7/11-6/12 21 $382,929 7/12-6/13 5 $251,594 7/13-3/14 5 $133,265 4/14-3/15 3 $98,901 0 20 40 60 No. of claims No. of claims

  • 3. 241 W. 56th Ave · Denver CO 80216 · Phone 303-297-9779 · Fax 303-297-0493 The above data reflects significant improvement year-to-year in both the number of incidents and the severity of the claims. The data reflects continued improved numbers for three policy years. The year 2012 is when I was designated as a full time safety manager. Prior to 2012 I was responsible for the Human Resource function and Safety, which did not allow me to focus 100% of my time to safe operations. Management’s confidence in the safety program The owner of US Transport Inc. has expressed confidence in the safety program in two ways:  US Transport Inc. changed from a guaranteed insurance product to a captive arrangement in 2012 due to confidence and actual experiences with the safety program. A captive insurance program provides for a potential dividend if the claims are favorable versus underwriting estimates. The safety department is no longer an overhead to the company, but a profit contributor. The captive arrangement reflects dividends as follows: Policy Year Dividend Loss Ratio 7/08-6/09 not applicable 127.7% 7/09-6/10 not applicable 68.2% 7/10-6/11 not applicable 187.8% 7/11-6/12 not applicable 134.9% 7/12-6/13 $178,000 58.0% 7/13-3/14 $276,400 26.0% 4/14-3/15 $374,000 19.4%
  • 4. 241 W. 56th Ave · Denver CO 80216 · Phone 303-297-9779 · Fax 303-297-0493  The owner of US Transport Inc. was comfortable with the safety program and it’s potential that the deductible was doubled in 4/1/2014. The recent lower claims costs illustrate a savings of premium with a higher deductible. Insurance rate per mile The insurance rate per 100 miles for the trucks has steadily decreased year-to-year which reflects the lower claims costs translated to lower insurance premiums per truck. US Transport Inc. has increased the number of company trucks year-to-year from a low of 90 units in 2012-2013, to a current level of 150 trucks for 2014-2015. The data is as follows: Policy Year No. of Trucks Rate per 100 miles/truck 7/12-6/13 90 $3.27 7/13-3/14 135 $3.30 4/14-3/15 150 $2.97

The data is as follows: Policy Year Case Rate 7/09-6/10 7.14 7/10-6/11 6.50 7/11-6/12 14.17 7/12-6/13 5.28 7/13-3/14 5.57 4/14-3/15 3.78
  • 6. 241 W. 56th Ave · Denver CO 80216 · Phone 303-297-9779 · Fax 303-297-0493 CSA score for Hours-of-Service A principal reason that US Transport Inc. installed electronic logs in trucks was to decrease the CSA score for Hours-of-Service. The citations received for hours-of- service reflected 65% due to form and manner violations, (addition errors, missing data, logs not signed, incomplete logs, 7 days of logs not maintained). Using an electronic device removes the form and manner violations. I was responsible for:  Presenting the benefits of using electronic on-board equipment to management and the owner.  Advising the owner and senior management of a comparison study of the different vendors’ benefits, reference reviews, and costs, assisting in a decision.  Planning the installation and training.  Monitoring the installation.  Preparing the process documents for drivers and managers.  Monitoring and reporting to management the daily features of the program (speeding, hard braking, equipment failure, hours-of-service violations).  Upgrading the systems from keyboard technology to touch screen systems. The CSA scores improved as follows: Period Value Threshold March 2013 76.6% 60% June 2013 84.1% 60% September 2013 79.2% 60% December 2013 73.5% 60% March 2014 65.1% 60% June 2014 54.1% 60% September 2014 67.0% 60% December 2014 62.0% 60% March 2015 52.0% 60% (currently satisfactory) The actual score is currently satisfactory due to using electronic logs, educating drivers of the importance of legal logs, and continual review and supervision of the process.
  • 7. 241 W. 56th Ave · Denver CO 80216 · Phone 303-297-9779 · Fax 303-297-0493 Cameras I was responsible for US Transport Inc. installing cameras in the tractors, service trucks, and management vehicles at the end of 2014. It is documented that 80% of accidents involving a commercial vehicle the CDL driver is accused of fault. After all of the evidenced is presented, the CDL driver is at fault 21% of the time. The cameras will assist US Transport Inc. in defending the CDL driver from erroneous claims. The cameras are changing the driver’s behavior with: slowing down when turning; using seat belts; not using hand-held phones or devices; and increasing their following distance. The cameras have reduced claims when evidence reflects the fault of the other driver. To-date, for 2015, one claim has been denied of approximately $5,000.00 in costs. Subrogation Insurance subrogation is handled by me. Doing subrogation in-house increases the funds received by 30%, (the charge from the TPA for subrogating claims). In excess of $375,000 was received in 2014 for two claims, producing a savings of $112,500. Contaminations and spills Controlling product contaminations and spills is my responsibility to train and monitor driver activity.
