Working with one client, PreVista analyzed pharma claims and identified a group of members abusing addictive drugs, linked to a network of ‘pill mill’ prescribers and pharmacies. We created this sample report to quickly identify and compare suspect members and prescribers:

East Bay Labs™ 3 Report filters and options: • Drug selection: A list of ‘controlled substance’ drugs in Schedules 2 and 3. • ‘Quick Picks’ to automatically highlight outliers: (a) Prescribers: high rate -- The annualized PMPY rate by prescriber for DEA Schedule 2 and 3 drugs. For example, the total volume of scripts / the total number of members seen by that prescriber for the selected year. (b) Prescribers: growth trend -- The trend in the annual rate for DEA Schedule 2 and 3 drug scripts, in the selected year vs the previous year – to show if the prescriber’s volume of Schedule 2 / 3 scripts rising or falling significantly. (c) Prescribers: total volume -- The total volume of scripts for DEA Schedule 2 and 3 drugs by the prescriber, for the selected years. (d) Members: multiple prescribers -- Shows members with prescriptions from multiple prescribers for DEA Schedule 2 or 3 drugs, in the selected year. (e) Members: multiple C2 / C3 drugs -- Shows members with scripts for multiple DEA Schedule 2 and 3 drugs, in the selected year. • ‘Table’ tab: Displays the results in tabular form with sort options. • ‘Map’ tab: Maps the results, e.g. the address of the members and prescribers. • ‘Graph’ tab: Graphs the result • Search: Sort/search to select member(s) or prescriber(s) PreVista™ Analysis: Oxycontin and Controlled Substances

Pharmacy claims: The client provided three years of pharmacy claims data and matching identity and address files for members and providers. This allowed us to map members and the ‘pill mill’ prescribers they were linked to. 2. PBM claim file spec: The client also provided the spec for the pharma claim file created by their PBM. 3. The FDA NDC Directory drug ‘product’ data file: this file provides details for >51,000 drug products, e.g. proprietary and generic drug names, the drug class, and whether the drug is a “controlled substance” with the DEA schedule number. • The SubstanceName, StrengthNumber and StrengthUnit fields can contain a series of values if the drug contains multiple ingredients. For example, “Cocet Plus” is a Class 3 drug that contains acetaminophen and codeine phosphate. For example, here are the values for Cocet Plus: SubstanceName = ACETAMINOPHEN; CODEINE PHOSPHATE, StrenghNumber = 650; 60, and StrengthUnit = mg/1; mg/1. When the values are extracted and paired, this translates to “Acetaminophen, 650 Mg” and “Codeine Phosphate, 60 Mg.” • When a drug contains multiple ingredients, addictive substance(s) are not identified. With Cocet Plus, for example, the file does not identify whether Acetaminophen or Codeine is the addictive substance. Individual substances were cross referenced to the DEA Schedule ID code, to identify the addictive ingredients in each product. The values are CI, CII, CIII, CIV, and CV, which were converted to numeric 1, 2, 3, 4 and 5 in the PreVista database.
